HEALTH AND SAFETY INCIDENTS

WHAT IS A MAJOR UNUSUAL INCIDENT (MUI)?

A Major Unusual Incident (or MUI) is defined by the Ohio Department of Developmental Disabilities in OAC 5123:2-17-02 as an alleged, suspected or actual occurrence that adversely affects an individual’s health and welfare or where the individual is at a likely risk for harm. There are 19 types of MUIs, such as abuse and neglect. When an MUI is reported, action will be taken to prevent further harm to the individual, an investigation will be conducted and a plan will be put in place to prevent future incidents.
