What is a Major Unusual Incident or MUI?
A Major Unusual Incident (or MUI) is defined by the Ohio Department of Developmental Disabilities in OAC 5123:2-17-02 as an alleged, suspected or actual occurrence that adversely affects an individual’s health and welfare or where the individual is at a likely risk for harm.
There are 19 types of MUIs, such as abuse and neglect. When an MUI is reported, action will be taken to prevent further harm to the individual, an investigation will be conducted and a plan will be put in place to prevent future incidents.

How To Report an Incident
During Office Hours:
If you need to report an incident (MUI) during office hours, please call 419-629-2419 ext. 207 or email UIR-MUI@auglaizedd.org.
After Hours:
- If you need to report an incident (MUI) after hours, dial our main number 419-629-2419 and press 4 to be forwarded to the on-call SSA.
- If you have an actual emergency, please hang up and call 911.

MUI Frequently Asked Questions
All providers who are certified to serve persons with disabilities, as well as all Auglaize County Board staff, are required to report MUIs. MUIs should be reported immediately, but no later than 4 hours after knowledge of the incident, by calling the County Board on-call number, 419-629-2419 ext. 207.
Providers should report possible MUIs to Auglaize County Board of DD (immediately or) within 4 hours for incidents or allegations of abuse, exploitation, misappropriation, neglect, suspicious or accidental deaths or when a provider has received inquiries from the media regarding an MUI. All other MUIs are to be reported no later than 1 p.m. the next working day following initial knowledge of a potential or determined MUI.
Reports of abuse, neglect, exploitation, misappropriation or death are filed in all cases regardless of where they occur and/or whether or not services were being provided.
